MANILA — The National Police Commission (Napolcom) Regional Office 5 uncovered major procedural lapses and neglect of duty in the police investigation of the September 20, 2026 crash that a killed Jao Francis Per Mancera, a public school teacher in Camarines Sur.
The initial validation, ordered by Napolcom commissioner Rafael Vicente Calinisan, revealed several major investigative and supervisory failures.
Investigators neglected critical evidence and key witnesses by failing to secure affidavits, obtain CCTV footage, and conduct sobriety tests on the driver.
Procedural errors, including the omission of the official investigation report, site sketches, and photos during inquest proceedings, caused significant delays in filing the case.
Additionally, the probe uncovered improper conduct and a lack of oversight, marked by the unauthorized release of the unregistered vehicle and personnel drinking inside the police station.
As a result, investigator PSSG Jaymar Sierra faces charges of Grave Neglect of Duty and Incompetence, while Station Commander Capt. Sarah San Buenaventura is under investigation for Grave Neglect of Duty under the doctrine of command responsibility.
Naga City Police Office (NCPO) acting director Col. Rizaldy Caballero also faces administrative action.
"Tamaan na ang tamaan. Wala tayong sasantuhin dito," Calinisan stressed, emphasizing that no officer will be exempt from accountability regardless of rank.
Napolcom Region 5 Acting Regional Director Leilani Vecina assured the public that administrative proceedings will strictly follow due process.
